Markus actually killed a void being; we can briefly see a thin black "leg" before the blackout (we can also see Markus taking half a heart of damage just before the blackout). In the world save, we see that there are 30 iron doors in that hallway, but it leads nowhere (however, some of the other iron doors lead to rooms with holes in bedrock exposed, which the void beings may have come in from). The blocks near the door being broken at the end really gives a sense that the void being is coming after Markus.

2:40 put 0.25x video speed and pause, and unpause until you see that he lost half heart only
